As to City planning, I think it's extremely important that people not try and be super unchangably accurate with their initial design. Unless you find a 900m circle of glass out there somewhere, something's gonna be in the way. Best to plan in stages, knowing you'll need to make changes along the way.
Where did you get the information that permissions to others for Cantina, Med Center is a third level skill.
Originally, I believe the ability to have cantina and med center was tied to grant zoning rights when it was in Civic policy 1:
Commercial structures are placed by players in cities. In order top lace a commercial structure, you must have been granted permission by the city mayor via the /grantZonningRights command. If the city is disbanded in the future, the commercial structure will remain.
I find it hard to believe placing a cantina and med center require more skill than a shuttleport, cloning center, bank.....
We were travelling that path a few weeks ago. One of our members actually started writing a Flash program to let others get on, Load up maps, Save and so on. I was working in Illustrator and Photoshop banging out layout after layout.
But we're not solid on a location yet, and where we end up is the most critical part. I can map out the geology of a reason (already wrote a program to take advantage of the new "/note" text-file creation command coming I assume with this patch), but even then it's an educated guess on what's possible. I'm hoping to map out the area using a house deed.
For now though, we have stuck with regions, with locations around the compass designated for areas. Best we can do until the Hall goes up. Then we know the area and can start planning.

Those are "no level skills"
Only a Master Dancer, Musician or Chef can place a Cantina or Theatre and only a Master Doctor or Combat Medic can place the Med Center.
Where I think the confusion might be is that you're reading the "City: Specializations" info under Civic Policy III. This isn't to allow placement of the structures. This is simply to provide an additional benefit to them. For example, I believe there's a 10% XP or healing bonus available in a Cantina with a Specialization: Entertainment District. However, that doesn't prevent placement of the Cantina, nor does it prevent the Cantina from providing normal healing benefit. This is a requirement for the placement of a Theatre, but that is also something that only a Master Dancer, Musician or Chef can place anyway.
That's always going to be an issue, but personally, we're not "Registering City" until we're at least a City. Not worth it. Everyone that would come to our City before the shuttleport is likely to either have stumbled upon it by accident or is coming to conduct commerce via invitation (we have many existing customers). That's not a "defense", but I don't think city griefing is going to be a problem much anyway. In order to entirely ring even an Outpost would take such a dedicated effort that it'd simply not be fun, even for the most jaded.
But if someone does come along and place, there's no reason why the Mayor or citizenry couldn't open a dialog with them. One of our chosen spots has gotten populated by like-minded individuals. We've already started talking with them about a possible partnership.
Nothing is permanent though. I suspect that even before Christmas, we'll be hearing about how people how to destroy their City Hall (they can't be redeeded... or has that changed?) because they got griefed, or didn't know the terrain or any number of reasons. That resets the tech tree to zero.
Which is why I want to complete this chart. Having a glossary for everyone to refer to takes away some of the early guesswork.

Thanks for the answer. This is my understanding
1. Cantina and med center can be dropped by Master Chef, Dancer, Musician with grant zoning rights which is now in Novice Politician.
2. In Civic Policy three you get specialization Entertainment District, or Medical district to gain the special 10% to heals.
