Confirmed.
At 91 speed, you will hit the cap with every move using a weapon of 4.5 speed or less.

Oh one more thing... is melee/ranged defense capped too? If so, whats the max on these two. I know block is capped at 125.

Mitigation most certainly does not stack. Melee/ranged defenses are capped at 125 just like the secondary defenses, and they are used in the to-hit equation as a direct subtraction from your accuracy mods + weapon range mods.

You really need buffs.
I went for the longest time without buffs and without armor, just using medic skills to stay alive (I was also master doctor at the time).
But you really can't beat using armor and buffs. Max out your mind, all your secondaries and put whatever is left into health.
Get a good set of armor and use buffs all the time.
