I was also having trouble and tried many times before reading this thread. After reading the steps here I tried dropping a vendor, initialize, and immediatly go to use vendor. Worked for me this time but I've got a feeling part of the bug might have to do with individual players connection speeds and how quickly the radial menu comes up.

The secret to initializing vendors is right up there with unlocking the FS slot.

I was having the same problem intialising a standard terminal vendor. Whenever I placed it, initalized it... and tried to use it... it would say it was not initialized.
This worked for me after trying everything else:
1) Create your vendor
2) Drop your vendor
3) Rotate your vendor that way you like it
ok get ready for some fast mouse movements 
4) Choose Initialize Vendor, and quickly select Initalize Vendor again and again (you should be able to choose it 3 times before the option goes)
5) Now Use Vendor

Only way this worked for me was to drop the vendor, no rotates, initialize it, then open the radial menu when initialize was STILL THERE, and slide the cursor up to use vendor... If I ever opened it a second time without using the vendor, it didn't work. I had to slide the cursor up while it was still there, right after I initialized it.

I also think I may have stumbled upon a contributing factor.
I tried placing a vendor multiple times with no success. I tried everything in this thread - still no-go.
Then, I tried placing the vendor further away from the walls of my house and bingo, no issues and initialized right away without having to do any of these workarounds.
This was with a bulky vendor.
So, something worth looking into - if you try to place your vendor too close (as in colliding maybe) to a wall or other item in your house, it may not work properly.

Initializing and then quickly initializing it again before the menu option goes away worked for me..

I was going through this thread trying each way......this one worked clicked initialize and then clicked it again before it disappeared.

Logging out (using diconnect) with the new terminal in my inventory did the trick! Thanks!

My problem seems different.
1. Created NPC vendor.
2. Placed NPC vendor in basement of PA.
3. Initialize and paid maintenance.
4. Vendor working ok, put items for sale (2 bags)
5. Next day, login, "Vendor cannot initialise"
6. Radial menu shows options of "working vendor".
Any help on this one?
I ticketed to CSR, they said this bug will be
fixed next publish. All items will automatically
be put into the stockroom.

Just sharing my experience.
I had the same problem..."not initialized" over and over again. I tried everything, the original trick (I would never get the initialize option the second time), not rotating, rotating, standing on my head with a salmon on my left foot reciting the alphabet backwards in Japanese...no joy.
Igot it to work by creating vendor, relogging, placing vendor, relogging, initializing vendor, relogging, then using. Worked first time...
