The only time I can sneak up on a Jedi is if he is in Theed or another big city full of people.
Get yourself a dancer's leotard (or something similar), remove your "Master Bounty Hunter" title from above your head, leave your weapon in your inventory, dont have 10+ buffs under your HAM bar and dont launch seekers right at the shuttle.
"Sneaky" is not defined as driving with a tank through gardens and small walls to approach your target on the shortest way possible.
Avoid honking horn when closing in.......

Nor you, your point? Anyhow, dont wear a bh title above your head is a pretty obvious one, i wear master politician. I also went rebel to hunt rebels (dont use this character for pvp anyhow) to give my marks another reason to think they might be safe (also helps for players that like to run into bases). Try not to dress in your armour, instead have all your armour on an equip macro on a seperate toolbar.

Pretty much can't sneak up on anyone due to the radar, unless you are in a big city with a lot of people around, or hunting around Mining Outpost on Dant, there are always people running around there, too hard to target everyone.
Blue dot's, more like any dot's on the radar!
I don't know if it actually fools anybody, but I just throw on the Novice Bounty Hunter tag. I'm messing around with a template so I'm not CL 80 anymore anyway.
Out in the wilds, I zoom out on my radar a lot. A good rule of thumb is if I can't see my mark on the radar, my mark can't see me. Check the lairs on the radar and zoom out to the minimum distance your target would need to see you on their radar then look for a path through the lairs. I've been able to get within 100 meters of my Jedi that way without being noticed.

It gives up scouting and alpha strike for sheer unbalanced surprise (unbalanced because the surprise might be yours). It's a bit Indiana Jones and here is how it works:
I stop 600M from my target. I eat my foods, start my duelist macro and hop back on bike. I bring up map , then ride in at full speed , zeroing on the group , I am now flying in at full speed and time the "store" of the bike at the right moment. If done correctly you end up in a full sprint just a few meters from the Jedi. The Jedi has zero preparation time, he might also be aggrod with lair creatures. Start with a KD to add to the surprise and them follow up with ADV crit, and torso, as Jedi gets up start laying states, paralyze, fire, poison ext. It's worked the lastfew times i have tried it with full temps .
Have fun if you try it. I also have done plenty of hunting and stalking so I don't want to get cirtizied for not doing it, just tyring something new that coincidentlaly has worked to perfection twice. The first time the Jedi had the lair aggrod and he did not have time to get away, the second time was against a full temp that has cloak and he had not time to use it as I used KD, stopping shot etc and stayed near him the whole time. I am convinced that the sheer speed of the attack produced the victories.

If your mark is in his town, you might shuttle in, but if you do make it look like you already know where your going.Hit thevendor mall for some shopping orbring afew (non BH) friendsto go run some destroy missions.Anything you might normaly do when your not hunting Jedi that might bring youto a strange town will help to make you look like another harmless passer-by. It may take a bit of time, but your mark will lower his guard toward you if you look like you have a reason to be there.
