Master Ranger and I've never foraged more than one item at a time either. I've never been off of Tatooine and I've done alot of foraging, but no multiple successes.

I've been a Ranger for at least a month now sitting at 4/3/4/4 still grinding away at the Survival xp. Anyway I have never gotten more than 2 items at a time and that happens rarely enough that if I hadn't been looking at my inventory at the time I would of assumed it never happened. I also have never foraged anything but what is listed in the original post no rare items of any sort.

I may be wrong but aren't we supposed to get random rare items when fishing as well? Never gotten anything there but fish yet if so. The lack of items in both of these maybe related if it's a bug so thought I'd mention it.
