I have used concealshot recentlyon only a couple 'big' mobs(Tusken Executioner, Axkva Min) and had no issues with getting detected(random bleed aside). If you both are absolutely sure you didn't miss once(it's tough to tell sometimes through all the floaty numbers), then there may be some new problem that has come up.

It is supposed to be two misses, but I have had it happen after just one. Though misses are very rare in PvE when prone with MR.

easy way to check if you miss... drag the combat chat tab out of the normal one... make it small if you want, just look for any white writing (learned this with commando to see if i hit with the FT)... also were the mobs moving around some??? with lag that could cause them to get into their agro range without you realizing it... also if maskscent breaks you can pick up an agro that way...
personally i see conceal shot being nerfed some, its not really right to be able to solo the endgame material without much risk on your part... be smart: single pull with a bleed,kite them, HS3 its alot quicker...

ok it was the bleeding thing ... how can I prevent my enemy from starting to bleed?

It was my understanding that reds could still agro you even with conceal shot if they detected you through your mask scent...right?

actually no one has a real answer to this question but there are a few theories. One is that the random bleed is tied into the wound chance and the higher the wound chance on your weapon the greater the chance of the random bleed. Therefore if you have a lower wound chance weapon you have a lesser chance of the bleed. I also suspect that after a random number of shots(1%per shot or so)a bleed will occur naturally as a means of keeping people from soloing most of the high level content in the game with conceal shot.
