Neat tool.
I have also gotten a lot of use out of photoshop and the "allow overhead map rotation" check box in the options pallet. I can run all around my town and take images with a non-rotating overlay map and then align them in photoshop later and align them to waypoints that appear both on my small bottom left terrain map and on the overlay.
It works out nice, because I can see terrain (which is a big part of my city). We have several plateaus and valleys that give our town character. Ourc civic center is on a plateau about 200ft above everything else.

In order to get detail on your terrain map, just up your global terrain detail and detail range.

Oh! Great tip on the terrain detail there. I tried using the terrain view and found the detail lacking. Instead I ended up going into the structure placement and figuring out exactly what the terrain looks like. That worked well for me since the main terrain detail was water. Used that to make this and this as well as a map for another city. The first one took me a whole day to do, but when I went to do the other city, I was able to do it in under an hour for the terrain and a half hour for the houses. Without using this tool.

Maybe I missed it, but is there a way to remove an item you dropped without starting over?

You didn't miss it, I forgot to mention it. Yes, you can just drag it back over the tool area and drop it there, and it will disappear. I'm thinking of adding a few keybindings too, like delete to remove it without doing that. Maybe a trashcan icon as well. Or a wrecking ball I suppose would be more appropriate.

How has the response been? As far as loading and such.. Both initially and as the pieces are put in place.. I'm sitting across ethernet from the server, so don't really get to experience it as everyone else does.
