like DigiDante said, you need to have been fighing for some time. you need your health to be just above 0, and you need your enemy's heal to have been worn away quite a bit as well.
anyone who says Last Ditch is a "one shot" or an "i win" button does not know what they're talking about and is only trying to nerf a very valid ability.
While in some manner you may be correct, consider all aspects and situations please.
In large scale pvp, this post holds true and it will be unlikely that you can time your being attacked so your health being reduced enough to land a one shot damage LD.
However as a bountyhunter, your ensured the first attack.
So using this tactic as a BH
Find your target;
having a CM poison,fire, etc on you for health reduction ( no disease as the wounds will only incap yourself during the LD )
Drinka + 200 ranged accuracydrink ... dur 1 minute ( can't remember the name of it )
once health is within 25-50 range remaining ( using a 502-1163 dmg scatter 361 base dps 650 mod )
I have landed a shot at 6k dmg for a one shot incap on a jedi knight.
keep in mind you must examine the target and see if they have Aura up or Valor
Wait for one or the other to wear off if they do then do the steps above and your nearly ensured a quick victory

Nope, I'd say he's well over the border on that one.
Poor programming does not equal players exploiting. So what if you have your friend beat you up and then you target a Jedi for a LD, there is a good chance you will still miss and if you do you are definatly toast.
