I am alittle confused reading the questions Kwee, might be that I am tired, but I will try and answer.
I think the idea of bodytatoos and some sort of disguise kit sounds fun. We also need a bigger hair/fur/horn/frill colour pallet and new styles. And I would love to be able to change the fullness of lekkus.
Maybe also make it possible for IDers to invest more points into the profession and become an infiltrator kind of type, who can and will be involved in the GCW.
I would like to keep helping people to look the best they can and just how they want to, if we also can add some spice by being able to be involved in the GCW without having to have to fight it would be great fun

Additional Idea not in SOTP: changing last names. No potential for griefing and surprisingly very in-demand. I had a client today think that ID's can already provide this feature (not true, but shows that people think we should have this ability). Repeat business too (divorces and remarriages). The only problem I see is people wanting names changed to dirty things, but hey, they can do that in character creation. It would also give wrongdoers a second chance, after a CSR removes an offending last name, to get a new last name.

My addition for the ideas part: let us change the caps for first names, but not the actual characters themselves. I.e. LeEtDoOd -> Leetdood.

Okay, my 2 electron's worth.
1) It's an even draw between a more full hair color range including punkish colors, and body/facial tattoos for other races. I have so many requests for both. I'm uncertain about least return value at this time.
2) As I was driving home to day was thinking about this of all things. One of the ideas that I have seen tossed out is to have appearences degrade over time (adding weight, loosing muscle, etc), requiring that the person visit an ID to get back into shape. That could be expaned upon to include aging. I have yet to have anyone ask for an age enchancement. If people visually got older as time passed, there would be a call for face lifts. Secondarily, (and this is what was actually going through my mind) was what if the whole cloning process were just a little off. Every time a person cloned, a random modifiable attribute was given a random value. Not only would it ensure that we had a constant supply of customers, but the RP value alone would make it fun. "Oh dear, I guess the left you cooking too long, well we'll get that extra weight off of you in no time!" 
3) The Image designer is one of the hearts of the role play aspects of this game. It ranks right up there with the Artisans who make clothing; it's all about looks. Our profession(s) serves no other purpose other than asthetic. For example, if I give someone muscles, they do not get a stat boost. Our mods don't booststats, increase weapons skill, keep you from dying, etc.We make people feel better about themelves and therefor they have a better time in the game.On that note, I get a lot of repeat customers solely because even ifsomeone has worked for the "perfect" character, after a while of looking at the same thing all the time, change is nice. I keep 3/5 cards on all of my customers. I record thier base line attributes and keep a record of every mod I make. They know and appreciate this, so when they come for a special party look, they know I can instantly return them to their everyday look.They may have a cycle of looks, just to have variety, and I can put them to what they want easily. Most of the changes though, to be honest, are"hair" styles, makeup, and markings.To recap, the ID is one of the prime aspects that keeps this game from getting stale visually.

2. Yes.
More hairstyles. Like Biana said, having a base hairstyle would be a lot cooler. It would allow for nutty amounts of creativity.
I also think there should be a simple UI that allows you to actually cut the hair (and color individual parts - highlights) rather than just change it, thus giving the profession a lot more depth. Basically it would just be a transmogrified version of the graphic editor itself with simple rules to keep the graphics in check. It is laid out simply to not make it so it is a "RL skill based profession", yet allows for much variation.
Craftables is a must. Multi-Purpose dye kits (could be used to alter Tailor's Clothing or Armor, eh? eh?) in any color. I WANT GREEN HAIR! Don't forget Hair Restraining Devices and Lip Color Applicators. Tanning lotion? Inverted tanning lotion (think jacko)? SCISSORS? Equippable ones that we could use to viciously stab mean customers? I'd like that a lot....
In full agreement with the shifting image thing. Weight is a must. Hair is a must (at least allow lower level entertainers to brush/comb hair). Skin color (tanning) might be an interesting concept.
Last name change would be pretty cool too.
3. ID has the power to edit a person's Image. Image is a very very misinterpreted word. It's almost an understatement.
One question... how do you buff BF?
