We are having trouble seeing the target that's being painted by our Squad Leader. Are there any graphics settings that might prevent the target from being displayed?

Give these some pretty icons and put them in the first two slots of your toolbar. This will save you the trouble of repeatedly typing /watch or /listen until the game recognizes that 1m from target is within range. Once you are successfully listening to your entertainer, just type /dump to purge the loop.

Just a note on that second part.
If you wish to repeat macros the easiest way is to do this:
/watch;
/pause 5;
/macro macroname
Name the macro watchmacro.
You can call any macro by typing "/macro macroname". This also saves you from wasting valuable toolbar space.

It's considered a particle effect, so the Detail slider in your Graphics Options might have an effect. I recommend hunting something VERY large, like a kimogila or krayt, just to see the effect better; like all particle effects, it seems to scale with the size of the target's model. You'll see a very large 3D red "cross" above the target. Also, it takes a few seconds to really show up; the cross "pulses", going from large to small and back to large. You might just be looking when it's at the small end.
Will this work with the pass-through damage, so that the entire group could just have the squad leader targeted and fire through while the squad leader picks targets for volley fire and changes as needed?

Yup! Just remember that the Volley effect lasts for 30 seconds, but the ability itself is on a 45 second cooldown, so there will be times when you won't have a target painted.

I think I've said this before, but I will add again that Volley Fire (when used along with Korren's patented Pass-Thru Assist™) as been awesome at picking thePirates out from the rest of the riff-raffduring the Phase IV Sith raids atThe Village the last few nights.
I group my pet and use rally, then wait for my action to regenerate... go prone use volleyfire and sniper shot... its a good start on any combat...
lots of damage and if you time your next attacks you can get another snipershot before they get close with the volleyfire still working... (if you can get sniper shot to work)
