-Can't, just do it.
-Hard to say, because PvP goes much faster than PvE. Healing peoples wounds in the medical center after a big raid is also a good source of exp. Buffing your squad also helps.
-PvE is more of throw up, as you res people when you get there, and when you heal their damage when they need to take a break. I rarely go into the fight where there is a big group because it's harder to organize and see who you are trying to heal. Doctors also reduce downtime and make the group move faster with less breaks.
To level up in crafting, set up one of those crafting macros. And make nothing but advanced Biological Effect Controllers. You need something like 21500 Tatooine Fiberplast and 21500 Lokian Wild Wheat (assuming 5% critical failure). If you can't afford that, grind normal Biological Effect Controllers (takes 3x as long, though). Get a bunch of crafting tools, so you never have to stop.
High powered StimEs are the best way to get xp through damage. Woundpacks are the the most economical. If you can find a Creature Handler and a Combat Medic, and let the CM disease some big pets...you can max out all doctor in an afternoon.
For crafting:

I used 5 crafting tools so that I could keepcrafting constantly with no wait time for a tool to finish. With this ittakes about 4-5 hours to do theentire line of crafting. I also did Adv. Bioglogical Effect Controllers which meant purchasing approx. 20K of each resource (buy the cheapest stuff you can).
For med xp:
I'm still leveling and I don't really know any secrets myself. Just walk around popular spotswith a surgical droid and heal people. You might be surprised how few people actually go to the medcenter until they have extremely bad wounds. If you can, go on hunting trips/quests to advanced planets. People tend to get banged up alot on these places so even if yourgroup doesn't (or you don't have a group) you're guaranteed to heal a few people.

Find *anyone* with Novice Brawler (for their /warcry1)
Take your two crates...your buddy..and go find the lowest level mob lair you can find.
Kill off the spawn.
*DON'T ATTACK THE LAIR*
Have the brawler set up a macro where they just /warcry1 the mob lair.
Over and over and over and over and over and over and over again.
Heal them.
You will make massive XP.
And it will help if you have max mind stats...and some spice buff for your mind. Once you see how fast you will make XP..you won't want to stop...and the limiting factor will be your mind pool.
And if you can get hold of something better than Stim-B's...all the better.
Similar to the one with /warcry, but someone with Ranged Support II can set a macro that tumbles to prone, and activates itself, so that they do it over and over, which absolutelymutilates the action and health bars. Put a great number of Stim B's in the mix, and you've got a few levels of med in no time.

A few hunting trips to secondary planets with some groups netted me about a box of healing exp per trip.
For crafting, I did the grind with 4 crafting tools and regular BEC's. Boring as all get out but it had to be done.
