I did my ER, general surgery, chem rehab and a couple of other rotations in a small inner-city hospital in Cleveland (St. Mikes @ E.55th and Broadway, which is closed now.) It was a place where a lot of people without insurance came in because they couldn't afford to go to a family doctor, or even a clinic for ilnesses. I saw a lot of kids who, if they didn't get their shots there, would not get them at all.
The ER was busy enough there, that if something needed to be done, you did it. One day I would be carring 'specimens' to the lab and changing sheets on the gurneys. The next day might be 12 hours of spinal taps, MVA trauma, and assisting emergency limb salvage surgery. The attendings were busy enough. The residents pulled as much of the load as they could get away with. Nurses almost never had time to sit and read the paper. I never asked another resident, a student, or a nurse to do anything I wouldn't do myself. If I was asked to do something and did not have a good reason not to do it, then i did it. It takes all of 2 minutes to draw up, give, and chart shots, if that long. By me giving the innoc instead of waiting for a nurse or EMT to do it, that ER bed would be able to be turned around anywhere from 15mins to a half hour faster than normal. That allowed the waiting room to empty. When the waiting room was empty, then we could all get a breather.
Besides that, there are always the times at the local school physicals and so forth where they have the bright idea of giving bosters and such. Given the option to get out of a day of geri/psyche rotation to go do physicals is well owrth it IMO.
