As a mayor and master performer, I can tell you this is a self-feeding problem. Even the largest cities aren't going to be able to compete with a Theed, Coronet orAnchorhead for active people at any moment. Some of the largest cities might start to enter the same class as Bestine on a slow day - but Bestine's cantina can get downright dead on that same slow day!
The lack of starport kills the ability of player cities to serve as hubs for travel. They are endpoints - thus people have to have business there to be there. As most player interaction rewards high volumes of people, people seeking player interaction tend to gravitate towards the existing megacities. Without being a center of travel or a center of activity, all a PC can hope for is to be a center of commerce - but this tends to be a "get in, get out" type proposition at best. Events help (a lot) but have a pretty short-term effect and you can't run proper events constantly.
It might be different if faction recruiters or bazaars were available for player cities. These are two "centers" that attract flocks of players.
