I was curious about what are the most effective ways people are finding to tank effectively as tkm.
I was thinking that if I ran in and got the first couple hits on whatever it is I'm trying to tank and then heal and then let everyone else in my group start shooting that I could hold the mob for at least a few more hits. but what strategy do I take from there. I know this is sort of a known issue, but I'm looking for some workarounds rather than just more complaints.

I set up a macro with cob and my doc abl. plus if anyone else in the group is a doc they can heal you up I was holding back 8 level 81 dewbacks using this. But I died in the end. thedoc that was my back up couldnt get to me in time. But i did hold them back for about 3 mins

I'm almost TKM with Master Doc. I was thinking of adding Pikeman 1 x 3 x for the intimidate, warcry and area knockdown. would allow me to do crowd control as well as tank, i was thinking...

Whether you're helping newbs and grinders by bashing bols or helping hunt for loot by drawing krayts and nightsisters to your being, crowd control is essential. A TKM without some crowd-effecting abilities is useless as a tank when the heat is turning up and your group accidentally aggros too many too soon. Everyone who takes tanking seriously NEEDS 1) crowd aggro/control skills, to pick up excess aggros or hold main aggro, 2) be able to helpkeep aggro by damage output, 3) healing, so that your group doesn't have to babysit you when you tank - they should be doing damage, not hold your hand.
TKM+Pike or Sword and some Doc I feel is essential - if you want to _tank_.
PS - I realize I'm starting to sound like a broken record, so I'll rest my case for a while.

I have found sword's Area Taunt to be much more effective at holding aggro than TKA's single-target taunt. I use that now and it works, but watch out -- you WILL draw a lot of fire from it. However, I was able to save a friend who over-aggroed about 5 storm troopers from certain deathblow (she was incapped before I could react) by doing area taunt. Of course they all turned and fired at ME, but heheh... she was able to wake up, self heal, and assist me, and I was able to hold them and tank long enough for her to do that.
With single-target taunt, I could've pulled ONE of them, and the other 4 would've DBed her.

And everyone who has been on Dath has seen the newb who area fires into a Bol congregation, and the 1337 CL80 guy who think that keeping that Pickets mission is a "good idea". 
Night sisters and rancors also have a tendency to aggro en masse, so for a shorter and more successful hunt (less running for the damage dealers, and you trying to keep up) you'd better have them groupeffect skills. I've also been running around with an ancient krayt behind me, while my compatriots finish off the original Krayt we were getting until we unexpectedly had another one spawn right upon us.
For the latter TKM+Doc will suffice, but maybe some scout would be nice though.
