I am almost a TKM (4-3-4-3) and a Master Rifleman, i am just woundering how melee/ranged defense actually works in PvP. Does it reduce damage taken or the chance for your enemy to hit ect.

Chance to hit.
The only modifiers that decrease damage I believe is the melee profession-specific toughness bonuses.

By how much does it reduce the chance for them to hit? Say for every 10 points ot so.

Its supposed to be 1% per point, but since its also possible to have over 100 accuracy, then its not quite as simple as having 100 melee defence makes you unhittable in melee (theres probably a 95% cap or something which no one has told us about as well.)

I've heard that your melee and ranged defense is directly subtracted from your opponents accuracy.

So i now have about 120 Ranged defense, does this mean a Master Commando can NOT hit me with his flame thrower (which has 110 accuracy at master)?

no, they just have a -10 when they shoot at you. Just like when you are 3 meters away you have a -15 (or whatever) on your screen.
