right now i am a mtka/4442doc and my plan was to go mtka/mdoc/scout1000. but i have started to think if i should drop doc for something else(tka/pikeman/??? or tka/swordsman???) All i am mainly trying to do some pvp/pve and the jedi trails/quests when they come out in pub 10. any ideas or opinions would be greatly appreciated.

Well, if you keep Doc you can heal yourself easily and last longer, but you will be limited to kinetic damage and light AP only.
If you add Swords or Pike you gain another damage type and AP2.
Really you just have to decide which of these is more important to you...

Here are a few builds I would suggest.
1. TKA/rifleman with exploration branch, you get meele and ranged attacks from this as well as some variety plus TN.
2. TKA/doctor, can heal and buff yourself.
3. Master brawler/TKA/swordsman, great meele PVE build.
4. Master brawler/TKA/pikeman (or fencer), great meele PVP build.

The big 2 things you get from master Doc/TKM are
1: self buffs
2: instant disease cures.
Those help a TON when fighting Giant Dune Kimogilas.
My suggestion is, if you know other doctors who play with you, or if you dont fight strong-disease enemies, go with TKM/some other combat professions.
If you hate waiting in line for buffs, love to fight Giant Dune Kimos, and are pretty much a loner, Doc may be a good idea.
Dont change your template in order to be better.
Have fun, then tweak your template as needed.
Even a Pikeman can be strong in PvP with the right accompanying skills.
