Combat Medic and TKA conflict with eachother. Combat medics specialize in ranged healing, whereas being a TKA, you want to be right up front tanking, like you're supposed to. My 2 cents...

For the choice between CM and Doc it would really matter on what you wanted to do. I had both CM and Doc, but eventually gave up CM. CM get ranged heals, but you must be fairly high up the CM knowledge tree to get the needed pts to use most of the higher ranged stims, where as Docs use the same med use skill as medics. Docs do get some better stims and wound packs, but depending on what you are after you may neot need them. Many of the overall advantages (or disadvantages) can be found in the CM forum, but ill list some of what they are.

CM: Advantages: Ranged heals, use poison and disease dots.

Disads: need to get combat med use to use them, not just med use (from medic). Poison and disease seems mostly useless until higher levels, and then seems geard towards PvP. Getting ranged combat support 4 sucks.

Docs: Ads: Higher stims and medpacks, can use most stuff with the med skills aquired with medic, can revive, cure poison and disease and enhance stats.

Disads: I do miss the range from CM, enhance packs dont do too much till you get to the higher level ones.

Anyway most of this is just opinion and everyone has thier own. I would recomend looking in the CM forum, there are a lot of doc/cm that post there and can give better accounts than I.
