They are all interchangeable between the melee classes. So you can be holding a polearm while doing a TKA special, etc.

Oetker - to answer your specific question I'd probably ensure you get 0100 TK (for CoB) for sure first. There are benefits both ways: If you go MTK first you get free armor but if you have enough credits to buy armor I'd go with the "other" melee prof first (after getting CoB). That way you have all teh offense needed to master TK more quickly.

Personally, if I were a new player I would do TKM first with medic and doctor along the way as the medical experience from soloing came in. While grinding your first couple professions, having innate armor and relatively cheap weapons (blaster, vks, etc) are extremely helpful. Grinding a second profession is always easier, having tkm just makes life much simpler in the end.
I am also TKM/Swordsman/Doc 4000 works fine (thought I think that I am not good enough / experiencedto solo ancients krayts...) I have tanked 6 ranzors same time - and you can imagine other team members faces when you walk out under dead ranzor mountain...
What kind food you are using? Honestly I haven't concentrated much to food issue.
Are you using power hammer?
How about PvP? Biggest problem in PvP is that ranged professions root you and after that you just stand there and heal yourself. Eventually ranged will win, because your mind doesn't recharge fast enough.

I use a LOOPED Bacta Infusion backed with a Spiced Tea (+101 mind regen for an hour)/Booster Blue cocktail. With Stim D's and Bacta Shot ready to roll enhanced with Rudy drink and +15 Healing eff mods.
I wear heavily modded clothing which consists of +25 polearm speed/+25 pole arm acc/+25 melee def/+25 ranged def. With various others.
Power Boost fueled with Bofa Treat/Kiwik/Thunder drink/Vegasparsine/Pikatta/etc. Nutrient Injection added too. Then mix in local Master Doctor buffs.
My main combat weapon is a 1010 max damage VL with speed PU's backed by Sinew speed stims.
With food & drink my Melee/Ranged def our both 350+ not counting COB.
Nothing is cooler then being mobbed and using area spin attack and watching all your opponents fall on their backs.
Thats how I roll for now till something comes along to screw this template up.
Solid, I have no problem with your statementsup to a point. I think Master TK should be able to stand on it's own as a tanking prof, and not require a second melee prof to make it the best tank. You are using area taunts (from Swords) and higher damage attacks, perhaps with higher damage weapons (also from Swords) to get and keep agro. You also have added melee / ranged defense from the 00x0 branch of Swords. These are just a few factors.
I also tried a variety of templates during the post-CU respec period... and I tried yours for a few days. I agree that it is enjoyable and I am tempted to use my token respec to go back to it but part of me really doesn't want to. I am currently MTK / Master Doc / Novice Pikes w/ 3.5 FS branches that give me +20 melee / ranged defense, among other things. I went back to Master Doc because suddenly there were no Master Docs around providing rezzes in hunting groups in tough places like the DWB, the Avatar Station, or the Necrosis Lair. True we have a few CMs in our guild but there was definitely a need for a Master Doc, so I went back to it.
Now, while I can tank an Ancient for a bit with the added defense of my healing abilities, eventually my Mind pool will drain, even with all the best foods, drinks, and spices working for me. Also, MTK by itself has so few moves (compared to other melee profs) it has become pretty boring. Add a Jedi into the mix and I become fairly superfluous, because their armor break supplants mine and they can heal the group almost as well, if not better in some respects. The only thing I am good for is keeping agro, a few buffs, healing wounds, and rezzing on those rare occasions when it is needed... but when itIS needed, it is essential to have a rezzer (well, almost, with all the rings, rez arms, and jedi rezzing out there, heh). Most of what I am good for is not related to TKA -- only the meat shield aspect of it. And that has simply become boring.
Ok, ok, my choice and my problem... but I do believe that TKA should be able to stand on it's own as a combat prof, without needing attacks and/or weapons or the added melee/ranged def provided by other melee profs. I see it as a serious problem. TKA is supposed to be the uber tanking prof and a moderate damage dealer, but it is neither unless combined with another combat prof (or at least pieces of them).
