Im a master TKA and working on getting flame 4 I also plan to keep novice medic. I am not interested in PVP at all and I dont want to be a doctor or high level medic, novice is fine. I bye my buffs and that works fine for me. What I do need help with is what should I dump the other skill points into. I could already solo enraged rancors before I even hit master TKA. I want to know what other skill I need and what the benefits would be. I would like to beable to handle most night sisters one-on-one (Elder excluded) and Kimos. I would also like to hold my own against a krayt even if I have to group with one other person. I was toiling between rifleman or fencer. I would have 46 points to play with. I could get rifle 4224. Let me know what you think.

Right now I'm a TKM/Master Brawler with 1/1/0/0 medic as my main focus. (I have some random scattered boxes in marksman, scout and artisan left over from hologrinding I haven't bothered to drop yet.) I do pretty well in PvE, though I haven't tried to solo a Kimo yet, I'm pretty confident I could do it.
Right now I'm wearing Comp Helmet, Chest, Gloves, and Boots (I'm human, go me) all with 80% kinetic, 69% base, sliced for encumb on all pieces. I've got my mind bar maxed (1100) and my Health and Action bars minimized (750). I also minimized Strength, Constitution, Stamina and Willpower because I could and still fit into my armor
I then split all remaining points equally between quickness and focus.
I use well made, regular old Vibro Knuck (no special loot in creation) that got a nice damage slice. I keep 400 Heal, 40 use Stim B's on tap. And utilizea combo of V. Brandy, Muon and Blue Milk to keep my mind going. Eat Synthsteak to help mitigate some extra damage, and ALWAYS ALWAYS ALWAYS have good Bio-eng'd Ryscate(sp?) in me.
Between my powerboost, the food and drink. And the extra points in Quickness and Focus, I can pretty much spam unarmedhit3 at will with almost no HAM penalties (ohhh my -31% to action weapon power ups help too)
I can easily handle anything Dantooine has to throw at me, including those 80% resistant to everything Force NPCs. I will be doing some Night Sister hunting later this week, guess that'll be my true test. Again, I haven't tried Kimo's but for the hell of it I did try to solo a Krayt. Outcome? Well I didn't kill him, but for 30 mins of battling away he couldn't kill me either. (Canyon Krayt, not ancient. The Juvi's I eat for breakfast 2-3 at a time.) I called it a draw.
A friend of mine has been trying to solo Krayts for a while now. His current attempt is basically Master TK/ Master Rifleman with some other skills thrown in here and there, I'm sure medic included. We'll see how he does. But IMHO, which profs you take aren't really as key as how you RUN whatever you decide to take.
I'm sure a MHS/Master Carb can be just as effective as me if takes the time to really get to know what works and what doesn't about his template.

Im currently in the process of regrinding my ranged proffesion, from pistoleer/fencer to a rifle/swordsman combo (eat my hammer fury turrets!). But TKM was always my staple for pve. Buffed, with 79 percent ubese armor i could solo everything up to a nightsister elder without much problem. I still have some problems with krayts solo, but when i have a master riflemen backing me up i dont have too much of a problem. As you said you dont do much pvp, so you dont really need a ranged class to do stun damage. I really recommend a tkm/doctor combo. Commando is all nice and dandy, but youll do more front end damage as a tkm then your flamethrower ever will.

The template that has worked really well for me in PVE with TKA was CH, just a few boxes helped a lot, though this was before the major pet nerf. With the ability to order special attacks though it may be worth taking a look at, a good BE pet with a nice poison and disease attack can go a long way in taking tougher mobs down fast. If you are looking for pure tanking ability you probably want to look at fencer as an add on to TKA though. The only I issue I can think of with fencer is that a lot of the nightsisters are 100% resistant to stun damage.
One more thing I can think to add would be master brawler. The 20m KD is nice for dealing with running mobs, and warcry 2, even though it was nerfed can help you avoid taking damage if you are tanking more then one mob at a time. Also intimidate 2's duration is nice, I always use it when soloing in PVE. Takes the 500+ damage hits from enraged rancors down to 200 or so.
The template I am currently using/working on is Master TKA, Master Fencer, Master Swordsman, the greatest advantage to this template is the ability to use 3 different damage types. I was with a small group a couple days ago in the Forced Labor Camp on Dathomir and had 6 Nightsister Guardians on me at once, I was doing fairly well when I switched up to my power hammer and used area attack 1, was getting an average of 2k hits per mob around me, only went down because the hammer I was using was very slow and they were pounding my mind hard. If I would have had a muon in my inventory I probably could have taken all 6 of the NS Guardians.
Currently I'm at TKA 0/0/0/1, Fencer 1/0/1/1 and swordsman 0/1/1/2 with master brawler at the moment until I need to drop it for skill points. I was wearing 80% Kinetic Composite, and had 2100+ doctor buffs with 2 +400 Brandies.

Rifleman is the only profession that you would have to master to get any benefit at all from it.... My stack will be Master Rifleman/ Master TK/ Novice medic with first Aid 1 block.
