Sounds like you are moving during COB. To get it to "stick" you cannot move at all, not even doing a "turn" with the mouse. If you do it will fail.
Sometimes even if you don't it will fail anyway. Just keep trying it (you have to stay still the whole time). Sometimes it takes 3-4 attempts to get it to stick.
Yes, that is silly to the point of near-nonfunctional, but, you can survive high level stuff if you can get COB to activate. Without it you are in trouble against greys.
