Can flex-form shirt or muscle shirt have these mods on them?
1.)+25 Injury/+25 Wound Treatment
or
2.)+18 Def. vs. stun/+4 melee def
or
3.)both mods... #1/#2 together on the shirt?

nope, flex-form/muscle shirt only has 1 slot for non-combat enhancements, so the best you can do is probably +17 or +18 injury/wound treatment.
for 1) you would need a wooly shirt or leather trim shirt (low-cut top and revealing top also available for females)
for 2) you need either a formfitting undershirt, lined workshirt, or reinforced work shirt

My master doctor wears +18 Injury/wound shirt (revealing top), and stun/melee leggings with +9 medical on them (I had some lousy medical tissues lying about and as long as they add up to +25, who cares where they are?). With the armor hole, you can wear normal pants with your armor. Ask your local tailor how you can get +25 medical AND stun/melee into the same outfit - there are many different methods.
