I generally charge a straight 1500 per point(per mod, i.e. if it has4 mods and each is worth 15 per mod, the final form is 30x1500)not including wasted tissues. And since I'm about 500 over what seems to be the average, I'll most likely not add extra for waisted tissue unless it goes over two wasted tissues.
