Lets get the obvious out of the way, are you using the Imperial sword, the one with the health bleed or are you using the gamorian axe from jabba's with mind disease??

Yes it does, as can other kind of hits too, for example hit3 or hits from areaattack3. It has also been observed from heavy damage dealing attacks from other professions, such as crippleshot from carbineer or unarmedhit3 from TKA. I think there is a general probability of the victim getting a minor bleed when hit hard enough.

It has nothing to do with the special, or even the profession. This is a widely recognized "feature" that suddenly started happening to all professions a few months back. Periodically, a small random bleed will occur on using most any weapon/special combination. There has never been a definitive answer on why this occurs (to my knowledge), but it has been suggested that the chances of this occurring increase the more damage you do.
My personal opinion is that it is connected to wounds. The "official" strategy guide made a connection between wounds and bleeding. That guide was based on a lot of beta information that never made it to publish, and this bleed/wound connection never really functioned. I think, however, that this means they had always intended that there be some connection between wounds and bleeds. As you probably know, when you get a bleed, especially a substantial one, you can end up with some nasty wounds to your secondaries. I think they had intended the opposite to also be true, that when you take a big enough wound to your stats, you will also trigger a bleed. It was just never working right. But I think that, at some point a few months back, someone noticed a + where a - was supposed to be (or something like that) and changed it, then suddenly it was working as intended. They just never mentioned fixing it.
So, yes. 2HHeadHit3 will cause a bleed every now and then. But then so do all the other specials most likely.
