Okay, the first template would be lacking in healing which has become very important in the game. You would be able to heal wounds with the 4xxx TKA and wouldnt get hit as much with Improved CoB, but in the first template I think the tka and Mbrawler would be a waste of skillpoints. I cannot find a template that I would want to keep that is similar to to the first one you mentioned.
The second option you had was very doable, however I would just leave it as: MS, MP, doc 4xxx, forget the x1x1blocks in Novice Medic as they do nothing for you when you are CL80.
