I have been a Squad leader first and foremost when I first created my toon. I have tried many things with it to try to obtain the BEST Squad Leader as possible. The following is what I came up with. I'm sure there are others but I have found this works awesome.
Scout 4044 - The requirements for Squad Leader + Harvesting... makes you wanted in group hunts that are after Hides or meats.
Marksman 0404 - The requirement for Squad Leader + Pistoleer requirement
Squad Leader 4444+M (Of course)
Pistoleer 0044 - These skills with a pistol that is made and sliced for Speed works great! /Fanshot supplies great damage for support to your group. /Meleepistoldefense2 is great for when soloing ANDfor playing Medic/Marksman protector in a group hunt. An agroo breakspast front line to a medic... you can KD them and allow the Medic to get away. And /BodyShot2 for everything inbewteen. You won't need to take the Accuracy branch because you can use /rally when soloing and /steadyaim when grouped. I find that I VERY REARLY miss with /rally. When grouped of course the restrictions on Harvesting and Dragging doesn't please the group. So I stick with /steadyaim when grouped.
Medic 0230 - All Squad Leader (in my option) should be very well rounded in healing as well. I found 0230 and a Doc that will make Stim Cs with 30 medical use or lower works GREAT! This allows you to /dragincapped out of danger AND with Stim C use... you heal far better than Bs.
Entertainer 0003 - As stated above... I feel a good Squad Leader needs to heal all. Having Scout already give him tents ... medic for wounds and entertainment for minds. This combined with a Robe with +25 Wound/Injurgy Treatment Mod ... +25 Entertainment Healing Top and +25 Entertainment Healing Pants makes for a +65 wound healing mind... which heals at a great pace for on the road.
You'll also need a Pet of some sort just to group with and leave at home so you can use your SL mods and skills when soloing.
Other things I've tried are SL/Pikeman - SL/Rifleman - SL/Musician - SL/CM... but... the above template I found helps in more situations than any of the others.
What have others tried?

I have pretty much seen every combination possible with master Squad Leader including the Entertainers.
There is no "best" Template as you should really pick the skills that fit your playstyle.
I for one have never ever been a novice medic (except for on a different server than my main). I don't want to have to concentrate on useing the healing skills. This is just my playstyle.

Interesting template for sure. But i always liked to rely on other people. I would build a list of contacts doctors, rangers, entertainers that i would email specifically to see if they could accompany one of my expeditions. I encouraged my group members to tip the doc for buffs, and rez's, and the same for any dedicated medic heals. The Entertainers always got a base amount in advance, typically on a trip to dathomir i would tip theentertainer 25-30k just for coming out with us.

I think its better to pair Squad Leader with one of the ranged combat professions personally. You have the Skill pts to pick up Master Marksman, plus Master Carbineer, Rifleman, or Pistoleer.

Im a master SL/Master Rifleman with 2-2-2-2 in medic and bare requirements in scout, this is more of a PvP build for me, so i can heal myself from bleeds, and drag players, but it also gives me ability to help my groupmates (when you go to theed, and LFD shows up, you want all the combatants you can get.)
I think it more so depends on your play style.... a PvE specialist would want more scout, maybe even ranger, whereas a PvP player wants more combat skills....whats your flava, is the more important question

I intend to go Master Squad Leader and Master TKA. It's gonna be heavy on the mind, though, might change it for something else if it doesn't work, but for sure an elite combat profession. Maybe Rifle.

I really like mine. I have two characters, one who is my main character (Master SL, Master Rifle, Marksman 4004, scout 4014, medic 2000, novice artisan). and my alt (Master dancer, master doc). This way I can buff my group with all HAM buffs, survey for resources with my main character for my alt's crafting, and heal myself when others cant.

How about
Master SL
Master Carbineer
Scout 4/0/3/4
Medic 2/2/0/0
That way I can heal bleeds and drag players...

Mine is more geared towards a pvp build and what im experimenting with right now is
Master SL
Master Carb
Marksman 0044
Scout 4004
Brawler 0400
Fencer 0010
basically im trying to get as much melee damage mitigation as i already have ranged mitigation 3
Medic is not needed because i pvp in groups and SL is looked after closely in group even though the
abilities may not work the best they should right now. I think after the revamp us SLs wont need medic
because we will have a lot of worth and be closely watch so maybe i will drop my melee mitigation setup and
go for max ranged offense stuff to deal as much pain as i can. This template works great for me though
