Pick up Combat Medic 4000.
Look at the video located here: http://forums.station.sony.com/swg/board/message?board.id=GCW&message.id=378010
This guy is Smuggler/commando/cm4000, but you could theoretically do nearly everything he does in the video.

I dropped my carbineer skills to pick up CM4000, but I loved having them when I did. Leg Shot is a great damage special, and it hurts to go back to Body Shot. I think a Carbineer/Smuggler would be a great combo, but I like the style of pistols too much to switch over.
Your ace in the hole is Last Ditch. NEVER go into a fight with the intention of using it. But, at the same time, always keep it in mind for when things aren't going your way (and try to make sure it hits with some +accuracy food). Make use of your state specials, and when overlap occurs (both Carbineer and Smuggler have snares, for example), do some testing to decide which special is best for you.
Then it's just a matter of engaging in PvP to become better and more comfortable with your abilities.
