Ive been doing this since last year. Most of the time I get a "hehe" or the Novice Smugglers usually start to turn back and forth ina confused state wondering how I gave them the secret smuggler's handshake! lol
I spent a while going down the command list looking for a gesture that could pass as a secret smugglers hand shake. The waf came the closest but it still looks like someone laid one and your fanning it away.
And I use the nod too much, especially when i'm in my boba fetti don't want to speak to you mood.

Then again, it's only been posted in the Smuggler forum, so not many non-smugglers will see it. Especially Devs

yeah you can do it this way.. make the macro and name it "SmugglerShake"
then type /alias SmugglerShake /macro SmugglerShake
now you can type /SmugglerShake for the secret shake.
then type /alias SmugglerShake /macro SmugglerShake
now you can type /SmugglerShake for the secret shake.

I think its like this
/wickedgrin;
/Emote Gives %NT the Secret Smuggler Handshake;
You might want to double check in the social, what the full comand for wicked grin is. Ill double check when I get on later tonight.

I still do it but usually I just /nod at them if we are in an open area with people around. Its not so secret anymore when everyone see's it 
But a little /nod of respect goes a long way.
