I suggest you check the weaponsmiths forum ( I will probably pinch their list later but not quite yet) they have a listing posted and stickied at the top of their forum.

There is also a handy* list in-game on the Holocron.

ctrl+h, customer service, knowledge base and do a search on "combat upgrade weapon levels".

If I remember later I'll take a note of the article number. doh.
