With skill mods you can get
Cover: 100
Which is supposebly "chance to take cover". Problem is, even with it being that high, 75% of the time you fail to take cover.

Yeah I know... I even have 2 different types of skill tapes that are useless: one is +6 to take cover and a +4 to cover. Don't know the difference however...

The take cover mod is useless and no longer exists. Cover exists..but...that whole thing seems BROKEN!

i use /takecover extensively on NPCs, particularly ranged ones.
I did fail 4 consecutive /takecover's last night, which irritated me, but since I don't use it while in combat, it didn't affect me at all.
being under cover appears to increase the number of shots you can get away with before agro. Rather than using 25 conceal shots to kill a NS Ranger, I go under cover and kill with 8-10 strafes. Less wear on my gun, and much faster. They almost never live long enough to agro.
being under cover has the biggest advantage against ranged NPCS. I've taken out entire columns of ST's (12-15) from cover without my ham bars moving-- you get hit so infrequently that buffed stamina/constitution keeps up with the damage.
my assumption is that a Take Cover AA increases your chance to successfully take cover on an attempt, and a Cover +x AA increases the effectiveness of the cover once you've successfully taken cover. Increased effectiveness provides more shots before drawing agro, and a greater ranged defense bonus after agro.

From my experience and from what I have heard, you're right the mods work. They just aren't all that useful to the average person. Taking cover increases your ranged defense. It will workagainst any ranged attacking opponent. So,some of the NPC's it's effectiveagainst are Nym's, Nyax, Marauders, and faction missions. For anyone maxed at 125 or greater ranged defense, it'll give no bonus. I do not believe that taking cover provides you any better chance for conceal than does going prone, nor does it provide you with any extra shots before you draw aggro. My $.01.
