Docommercial structures, such as med centers and catina's, add maintenance cost to the city itself? Or does the cost of these structures come out of the pocket of the person who placed them?

For the city no. That maintenance is paid by the owner of said structure. They are 50 per hour (Cantina/Med Center, unsure of Theater), the same as a factory. Structures like Shuttleports, Cloning facilities, (and hopefully city garages soon too!) cost the city treasury.
