Good question, I asked this about Cantinas and never got an answer. If I had to bet I would say nothing happens. The only check is probably at placement, after that it probably never checks. (like Merchant vendors). But I would feel safer if I knew for sure....

Hospitals seem to act just as houses do, you can declare residence and set up entry lists and whatnot.
And, you can also /transferstructure to someone else. I planted a Player Hospital in our City and then transferred it to another MD without a problem.
Not sure if it would work if I had tried to transfer it to a non MD or MCM...don't really want to pay to find out either

here's my take on how it SHOULD work.... a Hospital should be certified by a doctor/surgeon etc.... if you were to transfer to nonDoctor or drop the tree it wouldn't be certified.... Cantina's should be a different story... To place yes you should have Master Dancer/Muscian.... but not to retain.... Never heard of a Certified Cantina before. Also we need SOE to give Theaters and Hotels a purpose

It won't let you transfer to a non-MD or MCM, I havent' personally tested that, but the test center people said that was confirmed on test.
