Does making your town a Medical Center actually give you 10% higher buffs? Also do you have to be in the town or just a citizen?

Med Center does work and as far as i understand you don't have to be a citizen to use this specialization... although i think city specializations should really be limited to just "citizens" of that city 
But you do have to be in the town to recieve the bonus to buffs. Not sure on what the percentage of it is though.
