I just want to be clear on it since I am reading two things. Do I as the mayor HAVE to have Civic III in order toallow someone to placea hospitalthat is able to be placed with City Rank III? Do I need both?

I think what you are thinking about is the specialization. That does not happen until later but you can get the hospital at town level.
