Just finished my template , not sure if I should keep Smuggler with Dirty Fighting I.

This is my current template ...

I would like to do a bit of PvP and PvE. I have 700k heavy weapons xp banked up , but I dont think taking LLC or Flame up 2 boxes would help much. If you were to distribute the 11 remaining points in medic , where would you do so?

I'd go with the either first aid tree or the diagnostic tree. Possibly both. 2 boxes of each has served me well.
