Make sure you don't have two active missions in datapad. Two is the max you can have.

I am a 4333 rebel pilot doing the vortex missions. I died on the second mission for the guy in the dearic theater. I go back to talk to him and all he says is - something to the effect of - "you have your mission" my choices of conversation are "yes, sir I do" and "yes" ( some choice 
). I flew back to endor where the mission was and no waypoint spawned etc.

I have another space mission that I took in the interim - the first space mining mission. Could this have anything to do with the it? In my journal it does not list any active missions for the vortex.
anyone else hear of this?

Some tier 4 missions if you fail a leg of missions you may have an alternate path through mission check quest log before heading in from now on

That would be it.
Very common mistake, I wish SOE would put in a notice about that.

The issue has only come up since RotW, before then you only had duty missions and missions for advancement, and duty missions are usually just take and fly on the spot.
