There's a problem with the server's. What ever th eproblem is it's affecting the abiltiy to correctly display time of events. Could be why you see multiple attacks in one second, your bike warps 100m from you when you dismount and a slough of other problems.
My favorites is when the AI (artificial intelligence) does not respond via counterattacks. Kinda funny hitting a mob of lvl 85s to have them attack once per 10-20 seonds and even then only do 9 points each.

It's been mentioned alot.
The culprit is latency between you and the server. Even though your client says 40 seconds is up, the server won't know until a few seconds later (or has already expired you before your timer ran out)
If you're on a very laggy planet, your best protection is to activate AI sooner than usual. If I'm on a non-laggy planet, I'll start another AI 5 seconds before the previous expires, but if the planet is laggy, 10 seconds or more.

I've seen that too.. although not convinced that it's lag - as I seem to get incapped WHEN i do the next avoid incap. I think it might be related to not having enough action to complete the move - not sure if/what action/mind costs it takes to activate avoid incap, but I mostly see this problem when low on action.
Perhaps I can do some testing to find out if this is the case.
