OK, I have Hairstyle Customization I...

Where do I start? How do I use this in game play? The manual was useless in trying to figure out how to start out gaining XP in this part of the skill tree.

your best hope is to find someone else who wants to be an ID but that isn't so likely so find an Entertainer who wants master entertainer and sit down and rearrange each others faces. You can get good exp fast. Start with eye shadow , then when you level up enough start doing lips, freckles and horn color. Just try them all out get a hang of what exp they give and fast fast they use up your mind.

ONce you make Novice rearrange cheeks (which everyone has) for 125 exp.

How to do it:
Group with someone. Select imagedesign off the radial menu.
Or /imagedesign self.
