I think that the name Image Designer should be changed to represent the whole profession any ideas are welcome.

I used to be a Master ID, and thought to myself where does this name come from?

Here are some ideas I have heard:

Plastic Surgeon

Physical Artist

Body Sculptor

Personal Trainer

Personal Artist

Personal Adviser

There are also some ideas on skill requirements as well, I don't think its an entertainer profession but more of a medic/doctor profession, and possibly branch off one of those.
