LOL, I thought this was just troll/spam and egnored it, until I read kwee's reply.

I also like the idea of changing the "expert image designer" ( face forming 4 ) to "Expert plastic surgeon". or maybe just "Plastic Surgeon" I see no reason to say Expert being there is no novice plastic surgeon,

The fact is that I've never used the expert image designer title, because people would think I was a master image designer before I was. It just seems duplicative really. Plastic Surgeon I really really like!

The idea for the title had been sent in to the devs by that time anyhow

I didn't want to get anyone's hopes up until I heard whether or not it would be accepted. Plastic Surgeon is a title that came up fairly often as a suggestion for Facial IV in 2003.

And its a fun title, execpt for those times when people demand that you buff them because they think you are a doctor.
