I've had it happen three times, and /bugged it each time. It hasn't happened for ages though, so there's a chance they've hotfixed it on TC. I haven't been able to see any connection between the three times I reported it.

It has happened to me once when I was changing my hair colour. Now I'm constantly changing my appearance now that I'm stuck in that tent, so it wasn't the first time I did that, nor the last. There was nothing differentin what I was doing to all other times I was IDing.

I just had this happen during a stat migration of all things! I was soooo frustrated! The guy offered to pay me double to try it again...so that's what I'm doing now. But, it's not the money so much as it is the loss of 10 minutes + worth of experience because this is devastatingly PAINFUL to grind.

Happened to me 9 times in a row as a MID...
Doing holoemotes and jaw adjustment, kept putting the error up. Strange.

I decided to break out the alt and do some testing.
I can get this to repeatably happen.
When I accepted both right after the "Commit" and "Accept" buttons were available, it would happen everytime. In order for it to not happen, I would have to wait approximately 10-15 seconds after the "Commit" button became available to the ID'er. I could click the
"Accept" button on the alt right after I clicked the "Commit" button and the changes would go through.

It appears to have something to do with time, at least in the tests that I ran.

There might be several bugs all bringing up the same result. But it will need alot of testing and input to pinpoint them. If you encounter such a bug (or any other bug) please submit a bug-report in-game using the /bug command. Be as detailed and clear in what you did to produce this bug. Additional to that please post it in the daily bug-threads on the in-live-board. This way you help the devs to notice the bug and to find it's source(s), too.
Thank you
(and a merry Christmas, too)
