The password cycled while I was in the warren and I didn't get the FOUO disk. Where do I take the evidence & encryption disks too? Or do I have to do the whole quest again?

You don't need the FOUO disk.. just return the evidence to the captain in theed palace. When you enter turn left, next to the stairs.
