ie. August 26, 2003 saw West Coast servers Ahazi, Bria, Corbantis, Flurry and Sunrunner go live. (according to the info I found).

(2) Most Populated Servers?

ie. Which ones have the most people logging on at a time, on average, and which ones have the largest number of Player Characters in their data bases?

you could try the astromech stats... but I don't think SOE releases that info for financial confidentiality purposes.
