Actually, if you look through the dev tracker this has been answered definitively many times. As stated before to receive the rewards all you need to do is finish FS, do the paddy trials and train one novice box of Jedi.

My combat alt finished the Padawan trials to get the second slot but never even used a lightsaber and never got a novice box. On Ahazi-TC he wears the title of Jedi Elder and he can Force Ghost and turn blue. I picked spy as his first profession so I don't know if he gets the stuff from Obi's house. He's CL 80.
My weaponsmith is a crafting master/Jedi but only combat level 6 with a few Novice Jedi boxes. He converted to Jedi Elder with combat level 6. He can turn blue and got the stuff from Obi's house. I'll respec him to Combat Level 80 before Tuesday so he'll be a Combat level 80 Jedi Elder.
