I am on Kettemoor and I walked right up to the Captain and he gave me the quest no problems.

Is this the 1st time you've done the Quest ?

The problem is he gives a choice to unlock 1 of 2 branches. If you do the quest once unlocking 1 of the 2 branches. The next time Phase 4 comes around he will not give you the Quest again to unlock the other branch. Since you already completed the Quest last Phase 4.

No, this is my second phase 4

I did Melee Defense last phase 4, and this is my 5th Unlock,I walked right about to the Captain and he gave me the quest no problems, no trouble, and no questions asked.

The phase 4 quest is only broken for some people, sadly i'm one of'em

You should ammend your post. You can see with the liveliness of people affected you are the exception and not the rule
