Allakhazam has listed that this skill helps reduce our chances of being affected by a surprise attack. What does it REALLY do? I haven't seen anything do a surprise attack yet. Unless it is just a bonus to the first attack towards us that we didn't initiate, I can't imagine what this is for.
Please help me understand!

Certain (ranged) professions have special attacks that are considered "surprize attacks". When they use these you are considered to be "off-guard" and therefore have reduced defenses for that attack. Alertness is your chances of "spotting" the surprize attack and keeping your full defenses.

And it's a rating of your ability to dodge certain attacks, especially those of Riflemen when they are in the covered position.
