I am currently a master doc but I want to try new things and was wondering if I remained a 0/0/4/0 Dr would I still be able to buff myself with 89 med use buffs. I hate to give up Doc but have been wanting to gain more skills.

Nope. You need 0200 doc to be able to buff at all.

If i'm not mistaken you're also gonna need the wound/injury treatment row too if you don't want your buffs to suck hard core. But w/ those two lines you should be just fine on the self-buffing.
