It is just based on you "combat level"? And your combat level is based on how far you have gone in a combat profession?
Sorry for the dumb questions. I just need a little clarification.

They're based on combat level, yes. But they're self-only and have a cooldown timer of 60 seconds.

Everytime I've used Ruby Bliel its been bugged. Instead of lasting for 15 minutes+ and cutting the timer in half it lets me take a 2nd stim immediately and then wears off (Kinda like Havla used to do). Anyone else have this problem?

If the Ruby Bliel isn't bugged then this may Doc tip of the year. If it is bugged...well....sometimes being able to use a stim D back to back is just what the doctor ordered. We'll take what we can get for now.
I don't think that the new stim rules are outrageous. They obviously aren't intended for Doctor use. They aren't even craftable by docs anymore (I have mixed feelings about that). They seem intended to be used by that poor dumb adventurer that went out to do mission without a healer, stumbled across a disgruntled Durni which proceded to kick is **edit** causing him to burst run back to town not noticing that the durn was actually a Durni Knight and could force run after him. As the Durni pours out his fiery retribution on this poor (dumb) soul he is afforded the use of a Stim to ease the pain and prolong his inevitable defeat so that later he can say that he put up a good fight. I think that's what they're for.
They cost entirely too much to make or buy for them to be abused. Even if the timer was 5 seconds, I'd still be wary of using them too often. That would burn a hole in my already decaying pocket, not to mention it would make BE's what docs used to be economically. And we can't have that again, now can we?
Anyway, I think the heart of the original post addresses the feeling of uniformity doctors feel right now. The CU has made it harder to distinguish oneself. Doctor power has been diluted and our potency has been divided among every other combat prof in the game. /sigh
