If I put doctor 4030 in my template, how limited would I be by the low med use mod?
Specifically, can I use cure disease / poison / fire blankets that are efficient in PvP (do not require multiple uses and so on) with this lower med use rating?

Yes you can. In fact you can get Janta blood cure B's that match your med use and are especially effective.
However for my money you're better off with Doctor 3040. In my opinion, disease isn't a big enough issue in PVP to sacrifice the higher med use. Especially now that area cure B's can't be experimented to a lower med use. You'd be able to use the regular ones at 4030, but only area cure A's, I think. the B's are where it's at.

I'm a 4030 doctor. My cure diseases/poisons are like 20-30 points less then the MU90
