At the Doctor FAQ (http://www.iootnega.com/doctor/faq.asp) I read about how buffs work. I also used the buff calculator and stumbled on an unfamiliar concept, Enviroment mod. The explanation was: "Environment mods include things like buffing in a player Medical City, and such. Buffing in a Med city will give you a 10% bonus. Enter 110 as a value for buffing in a Med City." What is a Med city? Is it all cities with a medical center or what?

hakk,
It would seem that this bonus applies to player cities that have set their specialty to medical. When entering a player city I know that you always get a "you are entering (name of city, specialty)" message. I would wager that you get a 110 rating in those cities wiht medical as their specialty, not at all bad if you can get people to come out there for buffs and such, its pretty hefty bonus if you check out the calculater.
P.S. the politician skill necessary for this seems to be Civic Policy III, medical policies.

as i side note, i don't know that i've actually run into a city with a medical specalization.

Are their any Medical specialty cities on Naritus? Id like to test the buff bonus it gives.
Hmm...890 power packs, 110 droid, +25 clothes, bivoli +26%......plus 10% more for med city....Could be a really nice buff....Im already hitting 2500 per buff...would that make 2750?? Could be very neat.
