if i had a skill set of 0/4/0/4 could i make and use the revive kits? or would i have to have 0/4/4/4?

All you need is 0,4,0,0 to use the RezKits. Anyone with Master Doctor Crafting can craft them and experiment to lower med use...but I've found they are already pretty low to begin with.

but how much do people sell them for then?

Prices vary. I've seen them for 1k on the bazaar. I give some to guildies who can use them.
